To register as a new patient please call at reception, preferably with your medical card, to complete the necessary forms. Please note that you will need to bring ID (preferably photo ID) & proof of address with you. If you are an adult registering with a the NHS for the first time, you will need to bring evidence of your entitlement.
Registration will be with the practice and not a specific GP, although you will be allocated to the list of one of the GPs as your usual GP, who will be responsible for your ongoing care.
You have the right to express a preference to see a particular GP, which will be entered on your record.
